(1)Whether she has been informed of any threats levelled against a certain person (name and details furnished); if so, (a) when was she informed, (b) by whom and (c) what are the relevant details related to the threats; (2) did she take any steps to ensure the provision or procurement of security services for the protection of the specified person in light of these threats; if not, why not; if so, what are the relevant details; (3) (a) what was the rationale informing the decision to terminate the specified person’s contract and (b) will she furnish Ms B S Masango with the record of decision informing the termination of the person’s employment contract; (4) what is the (a) total monetary value and (b) the breakdown of the value of the severance package awarded to the specified person?
